4K Enrollment Information

This page provides information for families who live within the boundaries of the DCE district who are enrolling students in the D.C. Everest 4K program for the first time.

If your family lives outside the DCE district, you may enroll but your enrollment is pending open enrollment approval. Please note that based on our current staffing, in the January 2024 School Board meeting, D.C. Everest determined there is no space for any open enrolling students who are new to the district and require special education services for the upcoming 2024-2025 school year. Space determinations are reviewed each January for the following school year. Visit our Open Enrollment page.

Enrollment is held in February. Any child enrolling in 4-Year Old Kindergarten must be 4 years old on or before September 1 of the year for which they are enrolling. Families can choose from morning or afternoon half-day classroom options. Students can be a part of the 4K program for one school year.

Please note, 4K community sites have limited enrollment and applications are processed in the order they are received.

We encourage families to enroll their student(s) online. If you prefer to enroll using paper forms, please scroll to the bottom of this page where the forms are listed, or you may pick up enrollment forms at one of our 4K community sites. If you choose to pick up forms at one of these sites, please contact the location and schedule a time to pick up and return the forms.

Before beginning the enrollment process:

Check the 4K Boundary Map to determine which site your child will attend.
Have the following information ready:
- Household address and phone numbers
- Parent/guardian work and cell phone numbers, email addresses
- Student health/medication information
- Emergency contact addresses and phone numbers
Gather your child’s immunization records. If you do not have up-to-date immunization records available, you can visit the online Wisconsin Immunization Registry and download your child’s records by providing the child’s name and Social Security number.
Gather a copy of your child’s birth certificate. If a birth certificate is not available, you can complete Form 5111 F1 and submit the documentary evidence listed.
If custody has been established by the courts, a copy of the court order or placement papers must be submitted.
If you are enrolling more than one student, you will be able to enroll all students in one session (regardless of what grade they are entering). You do not need to complete individual online enrollment sessions for each child.

Before you enroll, please have your student screened.

Developmental Screening for My Child

The D.C. Everest Area School District will offer screenings through the use of the Ages and Stages Questionnaire-III and Ages and Stages Questionnaire - Social/Emotional.
After the child’s guardian completes these questionnaires, a developmental specialist will follow-up for an individual consultation.

Immunization Information

Immunization Requirements for All Grades

Wisconsin state immunization requirements for 2024-2025 school year are listed below. Students need the listed vaccinations by the first day of school.

Vaccinations are important. According to the CDC:

Vaccinations can protect your child from vaccine preventable diseases.
Vaccinations are very safe and effective.
Vaccinations can save your family time and money – reducing extended absences from school and/or work.
Vaccinations protect others – especially infants and preschool students.

DTP/DTaP/DT Vaccine (Kindergarten students): Your child must have one dose of the vaccine after age 4. If your child received all four doses before age 4, a 5th dose is required before Kindergarten.

Tdap Vaccine: One dose of the vaccine is required for students in grades 7 - 12. The vaccine is not required if your child received the Td/Tdap vaccine within 5 years of entering grades 7 - 12. Tdap vaccine protects your child from pertussis or whooping cough

MMR Vaccine: Two doses are required for students in grades K - 12. Your child must have the 1st dose of MMR vaccine on or after the first birthday and the 2nd dose before starting kindergarten.

Varicella or Chickenpox Vaccine: Two doses OR a history of chickenpox is required grades K - 12. If your child had chickenpox, see step 2 on the waiver form.

No vaccination needed.
Tell the school health aide that your child had chickenpox Hepatitis B: Three doses are required for all students grades K - 12 Waivers Parents have the option to decline any and/or all immunizations based on personal, religious and/or health reasons. If you choose not to immunize your child, check the appropriate waiver (Step 4) and sign the form

Hepatitis B: Three doses are required for all students grades K – 12

*Meningococcal Vaccine: One dose of Meningococcal vaccine (serogroup A,C,W,Y) is required for students entering 7th grade, and a booster dose is required for students entering 12th grade. Students are assessed for this requirement in 7th grade and 12th grade only. Current Wisconsin students in 8th-11th grade will not be assessed for this requirement until they enter 12th grade. A second dose is not required for students who received their first dose of MenACWY at age 16 years or older.

Waivers

Parents/Guardians do have the option to decline any and/or all immunizations based on personal, religious, and/or health reasons. If you choose not to immunize your child, check the appropriate waiver (Step 4) and sign the form. Immunization waiver forms are available online.

Please contact your student’s doctor or local health department to determine if your student needs additional immunizations. Schedule appointments to receive the immunizations needed or sign a waiver indicating what vaccine(s) you do not want your student to receive. Report the dates of the immunizations to the school health assistant.
